General Practitioners and Occupational Therapists

 

We've made changes to create a process that is quicker and easier for you and your patient:

 
  • you can now complete your patient's application online
  • you no longer need to recommend the type of permit
  • online applications are forwarded automatically to the local council.
 

The online functional assessment for the application does not take long. If you are familiar with your patient’s medical history, it will take:

 
  • around 5 minutes if the applicant has already started their application online before the consultation
  • around 10 minutes if you need to start the application for the patient in the consultation.
 

Medical qualification verification

 

To complete the permit application, you must:

 
  • be a General Practitioner or an Occupational Therapist
  • have an active registration with the Australian Health Professionals Regulation Agency (AHPRA)
  • provide your date of birth (this information will not be stored)
 

The applicant does not have to be a regular patient. If they are not, then you will need to verify their identity.

 

Required mobile phone numbers

 

To complete an application your patient or client or their nominated representative must have a mobile number to provide with the application. This is used for application correspondence and an application cannot be processed without one. If your patient does not have a mobile phone number, there is an alternate process you can find here.
